Please click below to view and print two required forms for your child’s registration. Print them, complete information as best as possible and sign. Bring both forms to pre-registration events (see home page) or arrive early on the opening day of YouthShops or your child’s first day. Or, due to COVID we are allowed to take your registration and family income form over the phone at 740-580-2065. Also, you can call this number if you need help with the forms.
Your child may not participate in any workshops without these forms being completed by the parent/legal guardian or via phone registration. We apologize for needing to ask you your income level, but it is required by our funding source. Without these funds, we could not offer the workshops for free. We need one registration form for each child in your family, but only one income verification form per family.
